A Case of Severe Gestational Hypertriglyceridemia

Abstract
Severe hypertriglyceridemia is a rare condition in pregnancy. During pregnancy, lipid levels increase due to hormonal changes. However, severe asymptomatic hyperlipidemia is rarely seen except in carriers of genetic disease such as lipoprotein lipase deficiency and dysbetalipoproteinemia. We report a case of severe asymptomatic gestational hypertriglyceridemia without familial history of hyperlipidemia. Hypertriglyceridemia was diagnosed incidentally after delivery while taking a blood sample to check her hemoglobin level. No secondary causes of hypertriglyceridemia such as hypothyroidism, diabetes mellitus, alcohol abuse, and use of oral contraceptive pill were found on physical examination and laboratory tests. Lipoprotein lipase level was 323 ng/mL and apolipoprotein C-II level was 0.4 mg/dL. This report reviews the complications and management of the hypertriglyceridemia during pregnancy. It is important that the early diagnosis and the appropriate management could avoid fatal complication such as acute pancreatitis.
